Demonstration grantee activities ran from February 2007 through March 2008. Findings were first presented in the Community Solutions: Lessons Learned from the My Medicare Matters Low-Income Subsidy Demonstration Program report which was issued in July 2008. In November 2008, this Tool Kit was released to serve as a companion to the report and to help community-based organizations who may wish to adopt some of the strategies used by our grantees.
