ANOCs and EOCs

• Annual Notices of Change (ANOCs)

  CMS has released final 2008 Annual Notices of Change (ANOCs) that Medicare Advantage and Part D plans must get to their members no later than October 31, 2007. The 2008 ANOC must be received no later than the end of October 2007.

• Explanation of Benefits (EOBs)

CMS released model EOBs that Part D plans are expected to use as of July 2008.

Evidence of Coverage (EOCs)

The Evidence of Coverage (EOC) is a lengthy document that lays out the actual terms of coverage under the plan. Plans must assure that members receive the 2008 Evidence of Coverage no later than the end of January 2008. CMS describes the 2008 Evidence of Coverage template documents in an August 17, 2007, Memorandum (73Kb PDF file).

At the Pharmacy - 2008

• Template Pharmacy Directory - CMS has released a template pharmacy directory for Part D plans to use (63Kb Word file).
• Model Transition Notice - CMS has also released the model transition notice (34Kb Word file) that Part D plans must mail to new members within three days of when they fill a prescription for a medication that is not on the plan formulary or that is subject to utilization management requirements, such as prior approval or step therapy.
